Glendale Community College District provides workers’ compensation insurance coverage in accordance with California Labor Code Section 3700 for all employees who sustain a work-related injury or illness.
What Is Workers’ Compensation?
Workers’ compensation is a state-mandated insurance program that provides benefits to employees who experience job-related injuries or illnesses. This coverage includes:
- Medical treatment related to the injury or illness
- Temporary disability benefits if time off work is required
- Permanent disability benefits, if applicable
- Supplemental job displacement benefits (for certain cases)
- Death benefits to dependents (in fatal cases)
Reporting a Work-Related Injury or Illness
If you are injured at work or develop a work-related medical condition:
- Report the injury immediately to:
- Your supervisor
- Human Resources at workerscomp@glendale.edu
- Company Nurse at:1-877-518-6702 (Available 24 hours, 7 days a week)

- Company Nurse will collect information on the injury/illness, provide injury/illness triage with a nurse and provide information on obtaining treatment. For any life-threatening injury or illness, please obtain emergency medical treatment first and then report.
- Human Resources will provide a Workers’ Compensation Claim Form (DWC-1), Medical Provider Network Information and an Employee report of Injury form.
- Submit the completed forms to Human Resources as soon as possible to begin the claims process.
Medical Treatment
Employees are entitled to medical care through the District’s approved Medical Provider Network (MPN) unless a personal physician or chiropractor was pre-designated in writing prior to the injury.

Reporting a Workplace Safety Hazard
Please report if you observe a hazard that may lead to an injury or illness. To report a safety hazard that is a basic repair (such as a water hazard, a slip, trip or fall hazard, burnt-out light, a broken window, a broken door, etc.):
